29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used.   a)  On and before July 22, 2013, employees were exposed to electrical shock hazards when using three extensions cords that had fraying and damage to the outer jacket and insulation.     b)  On and before 22, 2013, employees were exposed to electrical shock hazards when using a power drill that had fraying and damage to the outer jacket and insulation of the cord.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer.  On and before July 22, 2013 at the worksite; employees were wearing dust masks while dry sanding joint compound and had not been provided Appendix D of 29 CFR 1910.134.

On and before July 22, 2013, the employer exposed employees to electrical shock hazards by allowing them to use four relocatable power taps to power extension cords, battery chargers, and tools.
