29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:   a.  On or about July 24, 2013, the above addressed job site, an employee was exposed to an electrical hazard while servicing an energized electrical receptacle without safety related work practices being employed.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h):  a. On or about July 23, 2013, at the above addressed job site, the employer had not been developed and implemented a written Hazard Communication Program for employees using hazardous chemicals such as Hydro Foam Concentrate, ZEP Extractor Carpet Shampoo, and Swisher Degreaser.
