29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.  On or about July 17, 2013, in the production department, the Mitre-mite bottom stapler did not have a guard, exposing employees to a caught-by hazard.  b.  On or about July 17, 2013, in the maintenance shop, the Grizzly Drilll Press did not have a guard on the chuck, exposing employees to flying debris and drill bits.

On or about July 17, 2013, in the production department, three pedestal fans did not have continuous grounding because the ground pin was missing or not in place, exposing employees to an electrical shock hazard.

On or about July 17, 2013, in the maintenance shop, the electrical breaker panel was blocked by equipment and material, thereby preventing employees to access the panel in case of an emergency.
