29 CFR 1910.134(k)(6) as referenced in 29 CFR 1926.103: The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    a) 725 State Route 15 South, Lake Hopatcong, NJ 07849.    The employer failed to ensure that employees who voluntarily choose to wear N-95 filtering face-pieces were provided with Appendix D information to include but not limited to use, care, and limitations of respiratory protection.    Violation observed on or about 7/16/2013.
