29 CFR 1926.651(g)(1)(ii): Adequate precautions were not taken to prevent employee exposure to atmospheres containing less than 19.5 percent oxygen and other hazardous atmospheres: On or about July 13, 2013, during excavation to install a new oil gathering system at the Seminole CO2 Recovery Project, East Seminole Field, the excavator struck an existing 3-inch flow/injection line releasing a mixture of water, oil and hydrogen sulfide (H2S) gas, one employee was exposed to H2S at 84 ppm. Neither personal protective equipment nor any other protective measures were used to keep exposures of employee(s) to air contaminants within such limits specified in the "Threshold Limit Values of Airborne Contaminants for 1970" of the American Conference of Governmental Industrial Hygienists. This condition exposed employees to health hazards.

29 CFR 1926.651(g)(2)(i): Emergency rescue equipment, such as breathing apparatus, a safety harness and line, or a basket stretcher, was not readily available where hazardous atmospheric conditions existed or could reasonably be expected to develop during work in an excavation. On or about July 13, 2013, at the Seminole CO2 Recovery Project, East Seminole Field, employees were exposed to hydrogen sulfide concentrations as high as 300 ppm without a respirator that was adequate to protect the health of the employee(s). This condition exposed employees to health hazards.
