29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  An employee was exposed to electrical shock and burn hazards while using a Milwaukee circular saw which was plugged into a extension cord which had been cut by the saw and had a missing ground prong.

An employee was exposed to a fall hazard of approximately 17 feet while performing cleanup operations on top of the lower roof and the employer did not ensure the employee was protected from fall hazards through the usage of fall protection.
