29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.     On or about July 17, 2013, the employer did not ensure an employee cleaning up below the areas where roofing work was being conducted was in use of a protective helmet exposing the employee to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On or about July 24, 2013, the employer did not ensure employees reroofing the roof of a building were in use of fall protection exposing the employees to a fall hazard of more than 6 feet to the ground below.
