29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:     (a). Worksite located at 3765 W. Highway 76, Branson, Missouri- Employees were exposed to overhead hazards from elevated employees and materials that were being lowered down from the roof as well as materials that were laid upon the top of the parapet wall just above employees working below who were not wearing hard hats.

Worksite located at 3765 W. Highway 76, Branson, Missouri- Employees were exposed to a fall hazard in that employees were required to climb over the parapet wall in order to access the extesion ladder. The parapet wall was higher than 19".

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder�s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a). Worksite located at 3765 W. Highway 76, Branson, Missouri- Emplolyees were exposed to a fall hazard in that the ladder being used to access the upper landing surface did not extend at least 3 feet above the surface being accessed.
