29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about July 24, 2013, and at times prior, employees were exposed to falls greater than 6 feet while they were installing roof sheathing and working from a lower flat roof with no fall protection measures in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) On or about July 24, 2013 and at times prior, employees were exposed to fall hazards while using two portable ladders that were not extended at least 3 feet above the upper landing surface.  These included an extension ladder to access the lower flat roof and a step ladder to access the upper tower structure.
