29 CFR 1926.403(b)(1):   Examination. The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees.  On 7/24/2013, workers from JJ Restoration LLC were repairing a roof of a residential building located at 2504 Lucas and Hunt Road, St. Louis, MO.  They did not ensure an extension cord was free from recognized hazards.  The extension cord was connected to a circular saw on the roof, which was used to cut the roofing.   The cord had damaged or torn insulation near the connection plug and was wrapped with tape.

29 CFR 1926.501(b)(11):   "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On 7/24/2013, workers from JJ Restoration were repairing a roof of a residential building located at 2504 Lucas and Hunt Road, St. Louis, MO.  They were not utilizing fall protection while working on the roof.  One employee and Santos Garcia, the crew leader, were observed on the roof of the building without fall protection, exposing them to fall to the ground below.
