29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):    a) Front Yard, Center of Structure - Worker in an unprotected 11' deep trench preparing to remove a micro tunneling machine, on or about 7/23/13.     For the purposes of this inspection, the employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.

29 CFR 1926.1053(b)(1) Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed:     a) Front Lawn, Center of Structure - Worker in an 11' deep trench, used a 6' stepladder to exit and enter the trench  on or about 7/23/13.     For the purposes of this inspection, the employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.
