The working surface is slippery from the pooling water and algae. Employee(s) exposed to a fall hazard while inspecting the ammonia system.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   On or about July 24, 2013, the following deficiencies were noted on the OSHA Form 300 for calendar year 2013.  On or about June 25th, 2013, in the primary room, an employee sustained a back injury when he slipped and fell while trying to remove a product from the Sortex machine.  The injury meets the general recording criteria as detailed in 29 CFR 1904.7(b)(1)(iii).

On or about, July 24, 2013, the highest company executive did not certify the annual summary report for the year 2012.
