29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.  a) Jobsite: Greek Orthodox Cathedral, Roof - On or about 7/24/13, no fall protection system was used to prevent the employees from falling off the edge of the roof.  Employees were performing gutter installation operations within 1 foot from the unprotected low slope roof sides and edges.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access.  a) Jobsite: Greek Orthodox Cathedral, Roof - On or about 7/24/13, the portable ladder used for access/egress to the lower level roof did not extend 3 feet above the upper landing surface to assist the employees in mounting and dismounting the ladder from 15 feet above ground level. Employees were performing gutter installation operations.
