29 CFR 1926.62(d)(1)(i): Each employer who had a workplace or operation covered by 29 CFR 1926.62 did not initially determine if any employee was exposed to lead at or above the action level.    On July 19, 2013, and times prior to, employer did not initially determine employees working inside a building under demolition were exposed to lead.  This condition exposed employee to potential health hazards associated with lead.

An indoor electrical juntction was used as part of an extension cord to power the welder.    This conditon exposed employee to potential electrical hards.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification.     On July 19, 2013, and times prior to employees were using a welder that was not free from recognized hazards.        An open electrical box place on the outside of the welder was not fully enclosed.    This condition exposed employee to potential electrical hazards.
