29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a). At lot-23 on Dunson Way, employees installing roof decking on a 6:12 pitched roof were not protected by an alternative or conventional fall protection system to protect them from a minimum 10-foot fall hazard from the eave of the roof to ground below.

29 CFR 1926.1053(b)(13):  The top or top step of a stepladder was used as a step:  (a). At lot 23 on Dunson Way, the top step and top of the stepladder was used by the employee to gain access to the truss joist area where he was working on the roof facia as observed on or about July 26, 2013.
