29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) At Calhoun Health Care: Employees performing laundry, kitchen and housekeeping activities were not utilizing eye and/or face protection while using materials such as, but not limited to, sodium hypochlorite, NABC cpncentrate, sodium hydroxide and other injurious materials, to clean, disinfect and sanitize linen, surfaces and equipment.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) At Calhoun Health Care: Employees performing laundry, kitchen and housekeeping activities were not provided facilities for drenching and flushing the eyes and skin where an exposure to material such as, but not limited to, NABC concentrate, sodium hypochlorite, lime and other injurious materials existed. Employees utilized the materials to clean, disinfect sanitize linen, surfaces and equipment.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(iii): During use, containers for contaminated sharps were not replaced routinely or were allowed to be overfilled:  a) Unit 300: The employer failed to ensure that sharp containers did not exceed the over fill line. Nursing personnel were exposed to the hazards of blood and other bodily fluids while administering injections, IV's and other related duties.

29 CFR 1910.1030(g)(1)(i)(A): Warning labels were not affixed to containers of regulated waste, refrigerators, or freezers containing blood or other potentially infectious material   Calhoun Health Care Center: The employer failed to ensure that all receptacles utilized to dispose of comtaminated waste were labeled as biohazard or red bagged to indicated blood and/or bodily fluid may be present. Employees were exposed to HIV, HBV and other infectious agents while performing various assigned duties involving patient care.
