29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: At the time of inspection the extension ladder located on the northwest corner of the building was not extended 3 feet above the upper landing. Employees were utilizing this ladder to access the roof of the facility.
