29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  Dick Butler Roofing Co. had an employee exposed to struck-by hazards in the eyes from the use of a pneumatic gun without eye protection, on a residential construction site in Savoy, Illinois.   The employer did not provide employee safety training in order to provide the employees with the knowledge to recognize construction safety hazards, as well as the procedures to follow in order to minimize or eliminate those safety hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Dick Butler Roofing Co. had employees exposed to fall hazards without fall protection, on a residential construction site in Savoy, Illinois.    These fall hazards included:    a. 1st story - 9 ft., 7 in. and  b. 2nd story - 17 ft.  Dick Butler Roofing Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 312886971, citation number 1, item number 2a and was affirmed as a final order on June 11, 2009, with respect to a workplace located at 1956 CR 325 E., Mahomet, IL.
