Employees were exposed to an approximately 10 feet fall to the pavement below while performing roofing work where fall protection was not utilized.

Workers had not been adequately trained in how to properly install and use personal fall protection systems while working on a pitched roof where the anchor straps had not been installed according to the manfacturer.
