29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about July 29, 2013, employees doing decking work on the roof of a residential construction were not provided with fall protection.  Employees were exposed to a fall of at least 8 feet.

29 CFR 1926.404(b)(1)(iii)(C): Electrical equipment found to be damaged or defective was being used prior to being repaired:  On or about July 29, 2013, employees were using extention cords which had the insulation pulled back from the connector, exposing the internal conductors to damage.  Thus, exposing the employees to electrical shock.
