29 CFR 1910.30(a)(2): Portable dockboards were not secured in position, either by being anchored or equipped with devices which will prevent their slipping: (a) DiDonato, Inc. dba Peak Amusement and Vending at 100 Spruce Ave, Kremmling, CO 80459: On and before July 19, 2013 the employer did not ensure that a portable metal dockboard was secured to prevent it from slipping while loading equipment onto the Charmac trailer. The flat portable metal dockboard was placed across a gap that was created between the interior of the trailer body and its door when the door was in the lowered position. This condition exposed employees to a struck-by hazard when moving equipment onto the trailer.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks. (a) DiDonato, Inc. dba Peak Amusement and Vending at 100 Spruce Ave, Kremmling, CO 80459: On and before July 19, 2013 the employer did not develop and implement a written hazard communication program which at least described how the criteria specified for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks, had not been developed for employee exposures, such as but not limited: 1) Spray paints 2) Mineral Spirits 3) BEHR Weatherproofing Wood Sealer & Stain 4) Colony Gloss Varnish 5) ABS Cement Glue 6) Strypeeze 7) GT85
