29 CFR 1910.37(b)(1): 29 CFR 1910.37(b)(1): Each exit route was not adequately lighted so that employees with normal vision could see along the exit route:       On or about July 19, 2013 employees of Decker Electric, Inc. installing wiring for Koch Nitrogen Company, LLC and employees of Koch Nitrogen Company, LLC were exposed to hazards associated with emergency egress such as, but not limited to, inhalation hazards from ammonia, chlorine and NOx and slip and trip hazards.  On a total loss of power to the facility there was no emergency lighting along the egress routes outside of buildings to the evacuation points.

29 CFR 1910.119(h)(2)(ii): 29 CFR 1910.119(h)(2)(ii):  The employer did not inform contract employees of potential fire, explosion or toxic release hazards related to working on or near a covered process:      On or about July 19, 2013 and July 22, 2013 employees of Decker Electric ,Inc. installing wiring for Koch Nitrogen Company, LLC were exposed to inhalation hazards from chemicals such as, but not limited to, ammonia, chlorine and NOx.  The orientation training provided to contractor employees did not completely train employees of the hazards that could be encountered while working in the facility.
