29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    a) Loading Dock - Housekeeping employees mixed LOE Floor Stripper which is corrosive, on the loading dock.  The employer provided goggles but did not provide and ensure that employees wore a face shield, exposing employees to injuries.  Observed on or about 8/15/13.      b) Dietary - Employees used Scale and Film Remover which is corrosive to clean the dish machine, steamer, steam wells and ovens.  The employer did not provide and ensure that employees wore goggles and a face shield when they used the Scale and Film Remover, exposing employees to injuries.  Observed on or about 8/15/13.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Loading Dock - Housekeeping employees mix LOE Floor Stripper which is corrosive, on the loading dock, and the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body within the work area for immediate emergency use.  Observed on or about 7/30/13.      b) Units - Housekeepers use M95 stain remover for heavy stains, Shineline Floor Prep (an aerosol baseboard stripper), CDC-10 heavy duty acid bowl cleaner, and SC-200 cleaner, which are corrosive and the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body within the work area for immediate emergency use   Observed on or about 7/30/13.      c) Laundry - Employees change containers of Five Star Laundry Detergent and Rust Removing Sour, which are corrosive, and the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body within the work area for immediate emergency use.   Observed on or about 7/30/13.      d) Dietary - Employees used Scale and Film Remover which is corrosive to clean the dish machine, steamer, steam wells and ovens, and the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body within the work area for immediate emergency use.  Observed on or about 7/30/13.
