29 CFR 1910.179(j)(3): 29 CFR1910.179 (j)(3) :The items listed in subparagraphs (j)(3)(i) through (j)(3)(x) were not inspected for defects at intervals as defined in paragraph(j)(1)(ii) of this section or as specifically indicated, and all deficiencies were not carefully examined and a determination made as to whether they constituted a safety hazard. On or about 30 July 2013, and at times prior thereto, the employer did not inspect the 1/2 ton manual chain hoist on a jib crane located within the Tool and Die Shop for defects or deficiencies per the manufacturer's requirements. The hoist and crane are used to move parts in and out of the milling machine, with some loads up to approximately 150 pounds, exposing the employee to the hazards of being struck by the load.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment. On or about 30 July 2013, and at times prior thereto, the employer did not provide and maintain safe and ready access to the quick disconnect power switches on the wall for the MIG welding units, exposing employees to tripping hazards while accessing the panels.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. On or about 30 July 2013, and at times prior thereto, three employees were using a heat sealer and scale to repackage parts where the cords of each had been wedged between the work table and a metal brace causing the cords to be deformed, exposing the employees to the hazards of an electrical fire or an electrical shock from damaged wiring.
