29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer's:  a) Jobsite: 1100 Spring Garden St. - A fabricated frame scaffold with a height to base ratio of more than four to one (4:1) was not restrained from tipping by guying, tying or bracing.  Employees were performing bricklaying operations on the 6th working level, approximately thirty-six (36) feet above ground level. Observed on or about 07/23/2013.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.   a) Jobsite: 1100 Spring Garden St. - A ladder or other safe means of access was not provided to employees performing bricklaying operations on a fabricated frame scaffold. The employees were exposed to fall hazards of approximately thirty-six (36) feet above ground level when using the side frames as a means of access. Observed on or about 07/23/13.
