29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    In the shop, unused openings in electrical equipment were uncovered, exposing employees to electrical shock in the following two instances:    1) Circuit breaker panel 4(a), had openings created by improperly sized blanks installed over two vacant circuit breaker slots.    2) Plate bender on/off switch box was missing two knockouts.
