On or about 07/18/2013, at the job site - employees were permitted to work on a scaffold that was not fully planked, exposing them to a 13 feet 4 inches and 7 feet 4 inches fall hazards.

On or about 07/18/2013, at the job site - employees were permitted to work from a scaffold that was not fully equipped with all the necessary guardrails, exposing the employees to a 13 feet 4 inches fall hazard.

On or about 07/18/2013, at the job site - employees were permitted to work from the roof without any type of fall protection, exposing the employee to a 12 feet 8 inches and 9 feet 4 inches fall hazards.
