The endpoints at each upright were more than 48 inches (1.3 m) apart:   On or about 07/25/2013, the tubular-frame scaffolding approximately 15-16 ft high (from the scaffold to ground level)used by employees completing masonry activities at a property located at the corner of Graham Rd. and Route 8 exit in Stow, OH, was missing the mid-rail.
