29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces was not between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail crossing point of two braces were not between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform.  The endpoints at each upright were more than 48 inches (1.3 m) apart:   On or about 07/25/2013, the tubular-frame scaffolding approximately 15-16 ft high (from the scaffold to ground level)used by employees completing masonry activities at a property located at the corner of Graham Rd. and Route 8 exit in Stow, OH, was missing the mid-rail.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The portable extension ladder, used by employees to access the (tubular framed) scaffolding while completing masonry activities at a property located at the corner of Graham Rd. and Route 8 exit in Stow, OH, did not extended 3 feet above the upper landing surface being accessed.
