29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection:        For the dry cleaning facility at 212 Ridgewood Dr., Sebring, FL 33870, as observed on or about 07/31/2013 and 08/06/2013:    a) No locks were used in the isolation of steam line valves and air line valves during maintenance activities. Tags used were not securely fastened to equipment/piping, but instead hung loosely and could easily fall off when touched. Tags used also did not indicate the nature of the hazardous energy present and contained no warning legend relating to the hazardous energy.

29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) For the dry cleaning facility at 212 Ridgewood Dr., Sebring, FL 33870, as observed on or about 07/31/2013 and 08/06/2013: maintenance was performed on steam presses at the facility. Hazardous energy existed in the form of high temperature pressurized steam and compressed air. Multiple valves were present controlling steam flow and compressed air. No specific procedures were in place for hazardous energy isolation and lockout tagout of equipment.

29 CFR 1910.303(b)(2):  Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) For the dry cleaning establishment at 212 Ridgewood Drive, Sebring, FL 33870, as observed on or about 07/31/2013,  a flexible power strip was used to supply electricity to a Rinnai water heater on the production/dry cleaning floor. Use of a flexible power strip with industrial equipment is not within its intended, designed use.
