29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. WORKSITE: The employee, removing previously installed equipment from an elevated walking/working surface was not protected from fall by the use of appropriate fall protectin systems/devices. On or about July 30, 2013, the employee , while performing the above tasks, fell 12'-6" to the ground and sustained serious head and chest injuries from the fall.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. WORKSITE: The employee, exposed to fall hazards was not provided training in the recognition of hazards associated with falls or the methods necessary to eliminate or minimize those hazards.
