OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to:  (a) Prairie Farms installed an 8 cylinder compressor and added an intake for fresh air, but did not address the effects the change would have on the overall venting of the engine room and did not do any calculations to determine of the changed ventilation system would provide adequate air changes for all of the equipment room.  Prairie Farms did not follow Section 4.3 of ANSI/IIAR 2-1992, the International Institute of Ammonia Refrigeration Bulletin No. 111 06/02 or other nationally recognized safety guidelines.  (b) Prairie Farms had workers regularly working in engine rooms where ammonia systems were present, but had not identified the ammonia refrigeration piping and system components.  Prairie Farms did not follow the International Institute of Ammonia Refrigeration Bulletin No. 114 9/91 or other nationally recognized safety guidelines.  (c) Prairie Farms had emergency relief valves for the compressors on the ammonia system vented to the outside, but had not followed good engineering practices for the design and placement of those vents.  Prairie Farms had not followed the International Institute of Ammonia Refrigeration Bulletin No. 109 10/97 or other nationally recognized safety guidelines for the design and placement of emergency relief valve venting.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):    (a) On the roof deck equipment area for Engine Room #2 where workers inspected and serviced the ammonia system on a daily basis, the edge of the roof next to the ammonia equipment was not guarded.  Employees were exposed to a one story fall hazard.

29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    (a) Fixed stairs with a landing were not provided for access from a second floor hallway to the roof deck where employees went out on the roof deck daily to routinely inspect and service the ammonia system.  The drop immediately outside of the door was 26 inches.
