The employee was exposed to fire and explosion hazards.

29 CFR 1910.151(c):  Where the eyes or body of any person were potentially exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     At the worksite:  On or about July 31, 2013 and at times prior thereto, the employer did not provided a serviceable eye wash/emergency shower station on the deck of the suction tank near the caustic barrel area.  Employees were exposed to the hazard of chemical exposure.

OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm from a fall from heights:    On or about July 31, 2013, and at times prior thereto, the employer required an employee to work in the derrick of a drilling rig without the emergency escape device being anchored that will withstand 3000 pounds.
