As required by 29 CFR 1960.8(b): On or about August 5, 2013, in the General Surgery Department and the Internal Media Department (IMD), 110-V surge protectors were connected to each other to provide additional electrical outlets, exposing employees to hazards such as, but not limited to fire and electrical shock.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions. As required by 29 CFR 1960.8(b) : On or about August 6, 2013, in the Ophthalmology Department, energized electrical wires without a protective sheathing, were entering a junction box on the floor that provides power to the adjustable exam table. The protective conduit that was exposed to foot traffic had been damaged and broken away from the box, exposing employees to an electrical shock hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure. As required by 29 CFR 1960.8(b): On or about August 6, 2013, in the Ophthalmology Department, a surge protector was secured as fixed wiring to the wall where it was run behind conduit installed against the wall for another electrical outlet near the Lasik surgery table, exposing employees to hazards such as, but not limited to fire and electrical shock.

As required by 29 CFR 1960.8(b): On or about August 6, 2013 in the Oral Surgery Department, strain relief was not provided on the cord entering the plug on the Planmeca Pro Max machine, exposing employees to shock hazards.
