Manufacturer recommends when an operator is in the lift platform the palm switch button is to be engaged only when the operator in the platform is not able to operate the unit. Wheel chocks shall be installed before using an aerial lift on an incline.    a)      On or about 06-20-13, employees were operating a Terex Telelect Hi-Ranger 5TC Series aerial lift on a grassy surface without manufacture's recommended outrigger support pads; four 2 in. T x 12 in. W x 26 in. L boards were used by placing one under each of the four outrigger pads.  Manufacturer recommends each support pad to be 24 inches W x 24 inches L made of three layers of 3/4 inch plywood.  Two of the four outrigger support pads cracked and/or broke.
