29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    The employer does not protect each employee on a steep roof with unprotected sides and edges from falling when working 6 feet (1.8 m) or more above lower levels.  This violation occurred on or about August 2, 2013,  on the north side of the roof where an employee was exposed to a fall hazard of more than 6 feet when performing roofing activities without a fall protection system.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when exposed to falls 6 feet or more above a lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed   The employer does not ensure that portable ladders side rails were extended at least 3 feet above the landing when used to access an upper landing.  This violation was observed on or about August 2, 2013, on the north side and south side of apartment clubhouse where employees were exposed to fall hazards when gaining access to the roof with a portable ladder without the side rails extending at least 3 feet above the landing surface.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when using portable ladders by extending the side rails at least 3 feet above an upper landing surface to gain access.
