Kingsley Road, Garland, Texas; where employees were exposed to falling through rooftop holes.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    The employer does not ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level is protected from falling.  This violation most recently occurred on or about July 23, 2013, at a workplace located at 3000 W. Kingsley Road, Garland, Texas; where employees, who were working from a flat roof with unprotected edges, were exposed to the hazard of falling to the lower level.
