29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails:  a) 1056 Washington Ave. Albany, NY; On or about 08/05/2013: One employee was working from an aluminum 24'  extension ladder within 5 feet of the energized 220V service entrance, while a second employee held the ladder; employees were exposed to electrocution hazards.
