29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Jobsite, 730 Indiana Avenue, Glassport, PA:  On or about 08/05/13 employees engaged in residential construction activities approximately 20 feet above the ground were not protected from falling by the use of guardrails, safety nets or personal fall arrest systems.

29 CFR 1926.503(c)(3): Retraining had not been provided when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.  Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part:  (a) Jobsite, 730 Indiana Avenue, Glassport, PA:  On or about 08/05/13 the employer did not retrain employees when inadequacies in the employee's knowledge or use of fall protection systems indicated the employees had not retained the requisite skill.  Employees were working at heights up to 20 feet above the ground without utilizing fall protection systems indicating the employees, who had received prior training on fall protection, had not retained the understanding and skill to protect them when exposed to fall hazards.
