29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented :  Job site located at 9308 S 21st Street, Bellevue, NE 68147-Employees were exposed to a fall hazard of 11 feet to the ground level while engaged in roofing work without having any fall protection such as a guardrail system, safety net system, or a personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  Job site located at 9308 S 21st Street, Bellevue, NE 68147-Employees were exposed to a fall hazard of 11 feet to the ground level and had not had any fall protection training from the employer that would allow them to identify a fall hazard and the procedures to eliminate or reduce the hazard.
