29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident:    The employer does not report company workplace fatalities.  This violation occurred on August 2, 2013, when the employer failed to report the workplace fatality that occurred at 2124 Vintage Woods in Alexander, Arkansas, to the Occupational Safety and Health Administration.    Pursuant to 29 CFR 1903.19, within five (5) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with this standard by describing the actions it will take to ensure that any workplace fatality is reported ot an OSHA office within 8 hours of the occurrence.
