29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face , head or extremities was not used whenever necessary by reason of hazards.        At the fire wall area near storage tank 2177, on August 1, 2013, an employee was operating a Massey Ferguson tractor, model 231S, serial number 9681L04119, and was not wearing a seatbelt at all times while the engine was engaged.
