29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    a)  Roof, Residential Structure:  Employees, engaged in removing existing roof surface, did not have acceptable means of fall protection; thereby, exposing them to a fall hazard approximately ranging from 17' to 19' to ground surface below, on or about 8/6/2013.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed.    a)  Front Side, Residential Structure:  Employees, engaged in construction activities, used an aluminum extension ladder to access the roof area of the structure to which the side rails extended approximately 1.5' above the landing surface, on or about 8/6/2013.
