29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.  a) 336 Hackensack St., Woodridge, NJ: Employees were not protected by the use of GFCI outlets while cutting plywood with a circular saw, on or about 8/6/2013.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)   a) 336 Hackensack St., Woodridge NJ.: Employees without any means of fall protection were engage in roof work being exposed to a fall of approximately 25 feet, on or about 8/6/2013.  The employer TRC General Construction LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 562818, citation number 01, item number 01 and was affirmed as a final order on 12/28/2012, with respect to a workplace located at E. Prospect & Center St., Waldwick NJ 07643.
