29 CFR 1926.451(g)(1)(i):   Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system.  On 7/29/2013, employees from Jackson Custom Homes and Remodeling LLC were not wearing fall protection while repairing a residence located on 1042 Treeshade Drive, St. Peters, MO.  They were replacing vinyl siding on the side of a home.  They were on ladder jack scaffolding, which was 12 foot above the ground.  Neither employee was wearing fall protection.

On 7/29/2013, employees from Jackson Custom Homes and Remodeling LLC were repairing vinyl siding on a residence located on 1042 Treeshade Drive, St. Peters, MO.  They were working on ladder jack scaffolds with planks bridged on the center ladder for half a day.

29 CFR 1926.454(a):   The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On 7/29/2013, employees from Jackson Custom Homes and Remodeling LLC were repairing vinyl siding on a residence located on 1042 Treeshade Drive, St. Peters, MO.  They were working on ladder jack scaffolding.  The employees were not trained by a qualified person on how to recognize hazards and the procedures used to control or minimize hazards associated with the use of ladder jack scaffolding.

29 CFR 1926.1053(b)(16):     Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not  withdrawn from service until repaired.    On 7/29/2013, employees from Jackson Custom Homes and Remodeling LLC were repairing vinyl siding on a residence located on 1042 Treeshade Drive, St. Peters, MO.  They were using two defective or damaged portable ladders, no serial numbers.  Both ladders had damaged or bent rungs and one ladder was missing an extension catch.
