29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  At the time of the inspection, the employer had not certified that a hazard assessment for personal protective equipment such as respirators had been performed as required by this section. For employees working at field sites conducting sandblasting and jack hammering operations where silica was likely to be present the employer had not communicated the types of PPE to be worn.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
