29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section:    a.  On or about August 7, 2013, bank building, a lanyard was not worn by an employee who worked from a steel beam and was not provided with other means of fall protection which exposes the employee to falls from heights greater than 15 feet.
