29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:  a) On or about August 7, 2013 - at the jobsite located at 3153 S. Giles Avenue, Chicago, IL. - the employer did not ensure that the temporary electric system on the jobsite was free from recognized hazards, in that the electrical box was not kept clean and free of debris and accumulative dust, thereby exposing the employees to electrical hazards.

Giles Avenue, Chicago, IL. - the employer did not ensure that a temporary electric outlet used to power hand-held tools; including but not limited to drills, nail guns and band saws was not secured or mounted, thereby exposing the employees to electrical hazards.

The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.  a) On or about August 7, 2013 - at the jobsite located at 3153 S. Giles Avenue, Chicago, IL. - the electrical outlets used to power hand-held tools; including but not limited to drills, nail guns and band saws were not protected by ground-fault circuit interrupters or an assured equipment grounding conductor program, thereby exposing the employees to electrical hazards.
