29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:     a) Exterior Front of Building - An employee was hand mixing Lehigh Type 1-11 brick cement which contained Portland Cement and was not wearing eye protection, on or about 08/08/13.     b) Exterior Front of Building - An employee was observed cutting a stone paver with a DeWalt 7 and a quarter inch portable electric grinder, model number DW028493 and was not wearing eye protection, on or about 08/08/13.
