29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   The employer does not ensure that each platform on all working levels of the scaffold is fully planked.  This violation was observed on or about August 9, 2013, at the job site where employees were exposed to a fall hazard when working on a scaffold platform without it being fully planked.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels of scaffolds are fully planked.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  The employer does not protect employees on scaffolds more than 10 feet above a lower level from falling.  This violation was observed on or about August 9, 2013, at the job site where an employee was exposed to a 13 foot fall hazard when working from the third tier of a scaffold without guardrails.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working from scaffold platforms.
