29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Power Saw Shop, Forestry Development, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491:  On or about 01 August 2013, and at times prior thereto, the DeWalt 8 Inch Bench Grinder work rest was adjusted to a gap of 3/8 inch between the work rest and the abrasive wheel, exposing employees to hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Powr Saw Shop, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491:  On or about 1 August 2013, the DeWalt 8 inch bench grinder was adjusted to a gap of 1/2 inch between the tongue guard and the abrasive wheel periphery, exposing employees to hazards.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  (a) Power Saw Shop, Fort Apache Agency, POB 560, Whiteriver, AZ 85491: On or about 1 August 2013, material and equipment were stored within three feet in front of the electrical panel box mounted on the northeast corner wall of the workshop, exposing employees to hazards.
